(PenniesToSave.com) – Prime Air Flight 7598 was supposed to be routine. A 32 year old Boeing 767-300 weighing more than 100 tons, inbound from San Juan, Puerto Rico, on the third leg of a day that had already taken it from Cincinnati to Miami and back out again. Instead, shortly before 2 p.m. Eastern on Sunday, September 6, the jet overran Runway 30 at Miami International Airport, tore through a razor wire perimeter fence, crossed a four lane public road, and came to a burning stop roughly 1,300 feet past the end of the pavement [1][4][6].
Five people were killed. Five more were injured. Both pilots survived, and video captured a crew member climbing down from the aircraft and walking away from the wreck [3][4].
Every single person who died was in a vehicle on the ground [4][5].
That detail is what separates this from most aviation disasters Americans read about. Nobody who died had boarded a plane, bought a ticket, or accepted the risks that come with flying. They were cleaning crew workers in a company van and people driving down Northwest 67th Avenue on a holiday weekend. The airplane came to them.

What Happened in the Final Seconds on Runway 30?
Runway 30 at Miami International is more than 9,000 feet long, which is ample pavement for a 767 on a normal approach [4]. This approach was not normal.
Tracking data from Flightradar24 shows the aircraft climbed above the standard glide path used by jets landing on Runway 30, then dropped altitude to get back into position [4]. It stayed airborne deep into the runway, its left rear gear bouncing along pavement that was rapidly disappearing ahead. The plane touched down almost parallel to the ground rather than executing the standard flare maneuver, in which the nose lifts at the last moment so the rear gear meets the runway first [4]. By the time the jet reached the end of the runway, it was still moving at roughly 130 miles per hour [4].
The pilots never told air traffic control that anything was wrong before landing, according to ATC audio [4].
Francisco Diaz, a Boeing 747 captain and president of the Miami based Captains Council, was leading a class at the Boeing training center when the crash happened. He felt the building shake and ran outside. Reviewing the footage later, Diaz said it looked like an unstable approach, while noting that investigators will have many factors to work through [4].
Weather is one of them. BBC Verify found an active thunderstorm near the airport shortly before the crash, with winds gusting up to 26 knots, or about 48 kilometers per hour [6]. David Soucie, a former Federal Aviation Administration safety inspector, said the wind direction shifted suddenly during landing, giving Flight 7598 a tailwind. “You’re picking up speed instead of bleeding speed off at that point,” he said [4].
NTSB Chair Jennifer Homendy has been careful not to get ahead of the evidence. The agency says it is too early to tell whether weather played a role and that investigators remain in what she called the fact finding stage [5][6]. Both the flight data recorder and the cockpit voice recorder have been recovered and shipped to NTSB headquarters for evaluation [3][5][6].
Those recorders will answer the question hanging over everything else. A go around was available. Whether the crew considered one, and why they did not execute it, is the center of this investigation.
Who Were the People the Plane Hit?
The aircraft’s path after leaving the pavement is grim in its specificity. It first struck navigational aids and airport equipment, then hit a white 2012 Ford Econoline van on airport grounds. Homendy said the van was torn apart [3][6].
Seven workers from a contract cleaning company that services airlines were inside it [3][4].
The jet then broke through the airport perimeter fence, crumbling concrete barriers designed to keep cars out of the secure area, and struck a Toyota Corolla Cross traveling on Northwest 67th Avenue with three people aboard [3][4][6]. Ten people in two vehicles. Five died and five were injured, three of them taken to a trauma center in critical condition [1][2][4][6].
Homendy described walking the debris field and seeing deployed airbags scattered among aircraft parts and vehicle components [3][5].
I would describe it in one word, devastating, utter devastation. You can see on the paved surface, tire marks that continue through the grass. There’s debris everywhere.

A fuel leak slowed the recovery of victims. Homendy made the agency’s priority explicit, saying the recovery of victims came first and that access to the aircraft and the evidence could wait [5][6].
More than 60 Miami-Dade Fire Rescue units and roughly 200 personnel responded, with the first crews arriving within seconds [2][4]. Grace Mercado, riding in a car near the airport, said the sound was unlike anything she had ever heard and compared it to a shake [2]. Spencer Deno, mayor of the Village of Virginia Gardens, felt his house shake and drove toward the smoke [4]. One Boeing training instructor arrived back at the training center shaking, telling Diaz he had nearly been hit as the airplane came off the airport and into the street [4].
These were shift workers cleaning aircraft cabins for a living and ordinary drivers on a public road. Their identities had not been released as of the most recent briefings [2].
Why Doesn’t One of America’s Busiest Airports Have an Arrestor Bed?
There is a proven technology built for exactly this scenario, and Miami International does not have it.
An Engineered Materials Arresting System, or EMAS, is a bed of lightweight crushable concrete blocks installed past the end of a runway. It looks solid but collapses under the weight of an aircraft, sinking the wheels and dragging the plane to a stop, somewhat like driving into deep mud [4][5]. Mary Schiavo, a former inspector general at the Department of Transportation, put the mechanics simply: “When the airplane goes off the end of the runway, the wheels crunch into it. The plane is stopped” [4].
Sixty nine U.S. airports have EMAS installed [5]. The FAA credits the systems with safely stopping aircraft carrying nearly 500 crew and passengers [5]. Miami is not among them.
The reason comes down to how the federal standard is written. The FAA requires airports to provide a runway safety area past the runway end, and EMAS is the accepted alternative where an airport physically cannot fit that buffer. Where the buffer is deemed adequate, no arrestor bed is required.
Homendy said whether Runway 30 should have had EMAS will be a key part of the investigation [6].
An airport this surrounded by population, they need an arrestor bed system at the end of these runways to stop this from happening again.

Diaz made the same point from the cockpit side, noting that Miami, like many American airports, is now ringed by roads, highways, and buildings that did not exist when the runways were laid out [4].
Here is the uncomfortable part. Runway excursions are the number one incident type involving aircraft worldwide, according to the International Air Transport Association [4]. The industry has known this for years. Federal minimums were met at Miami, and five people on a public street still died. That raises a fair question about whether a compliance floor has quietly become a safety ceiling.
The counterargument deserves a hearing too. Arrestor beds are expensive, they require space that older urban airports often lack, and the NTSB has not concluded that one would have stopped this particular aircraft at this particular speed. Cost and siting are real constraints. They are not, by themselves, an answer.
Who Is Accountable When a Contractor Flies the Plane?
The fuselage said Prime Air. Amazon did not fly the airplane.
The 767 was operated by 21 Air, a cargo carrier headquartered in North Carolina with an operations center in Miami [3][4]. It is one of nine flight vendors that Amazon Air uses to move packages [4]. That distinction is not a technicality. Under FAA regulations, it is the operator who carries responsibility when an aircraft crashes, Schiavo told CNN [4].
She pointed to a case she litigated: the 2019 crash of a 767 into water in Texas, where the NTSB cited the pilots and the operator’s training program in its final report. Because the operating carrier held the certificate rather than Amazon, that carrier and its insurers bore the liability [4]. Schiavo characterized that earlier operator as a predecessor to 21 Air, though the two are separately held companies and that connection is her characterization rather than an established corporate fact.
Amazon spokesperson Kelly Nantel said the company was heartbroken to learn that five people lost their lives and extended sympathies to the families and everyone affected [4][6]. 21 Air issued its own condolences and said it is cooperating with the investigation [6].
The NTSB is not treating the contracting relationship as background noise. Homendy said investigators will examine the operator’s contractual relationship with Amazon, FAA oversight, crew experience and training, radar data, and aircraft performance, with separate teams reviewing systems and engines and a meteorologist assessing conditions at the time of landing [4][6]. A 32 person team is on site for at least a week, and the agency will not name a probable cause on scene [4].
When a household name distributes its shipping across nine contract carriers, the practical question for the public is whether accountability gets sharper or more diffuse, and whether federal oversight follows the brand customers recognize or the certificate holder they have never heard of.
What Does This Mean for the Average American?
Most Americans will never board a cargo 767. Millions of us live, work, drive, and send our kids to school within a few miles of an airport that was built on the edge of town and is now surrounded by it. That is the condition Diaz described at Miami, and it describes airports in nearly every mid sized American city [4].
The people killed on September 6 were not passengers. They were on a public road and in a work van. Perimeter design, buffer zones, and arrestor beds are not aviation industry inside baseball. They are local public safety questions, and they are decided by federal minimums and airport budgets that rarely get local scrutiny until something like this happens.
There is an economic layer too. The overnight air cargo network that makes fast shipping possible is flown largely by contract operators most customers could not name. That system is why a package ordered Sunday shows up Tuesday. It is also where the oversight questions now sit.
The immediate cost landed on travelers. As of Monday evening, about 167 flights to or from Miami were canceled and roughly 388 were delayed, with inbound delays averaging more than five hours [4]. The airport operated with two of its four runways during the Labor Day holiday weekend, one of the busiest travel periods of the year [4][6]. Runways 30 and 26L remained closed, and NPR reported officials saying they would not reopen until the investigation concludes [4][5]. Anyone who has been stranded by a closure like this knows the costs stack up fast in rebooking fees, unplanned hotel nights, and meals, which is one of the clearest arguments for building an emergency fund on autopilot before you need it. It is also worth going back through the resulting charges once you are home, because airline fees and hotel holds are a common place for billing errors to hide, and a simple system for tracking your expenses makes those easier to catch.

Final Thoughts
Investigators have the flight data recorder and the cockpit voice recorder. They have Flightradar24 tracking showing a high approach, a late touchdown, and 130 miles per hour at the end of the pavement. They have ATC audio showing no reported problem. They have a thunderstorm, a wind shift, and a tailwind. They also have a runway with no arrestor bed at an airport hemmed in by a four lane road [4][5][6].
What they do not have yet is a cause, and Homendy has been disciplined about saying so [6].

She also said something worth holding onto: “Our recommendations must be implemented, or we’ll be here again” [6]. That is the accountability gap in a sentence. The NTSB investigates and recommends. It does not regulate. Implementation falls to the FAA, to airport authorities, and to the operators themselves.
Five people who never set foot on an airplane are dead because a jet did not stop where the pavement ended. The next NTSB briefing may bring victim identities and crew status. The harder work, deciding whether federal minimums are actually sufficient for airports surrounded by neighborhoods, will take considerably longer and will happen well outside the news cycle.
